Pathology

English: Osteosarcomas are classified based on their histological appearance:

  1. Conventional osteosarcoma: The most common subtype, which includes osteoblastic, chondroblastic, and fibroblastic types.
  2. Telangiectatic osteosarcoma: Characterized by large cystic spaces filled with blood.
  3. Small cell osteosarcoma: Resembles Ewing sarcoma but with osteoid production.
  4. Parosteal and periosteal osteosarcoma: Surface osteosarcomas that arise from the outer layers of bone.

Clinical Manifestation

English:

  1. Early stage: Symptoms include localized bone pain, swelling, and tenderness. Pain may worsen at night or with activity.
  2. Advanced stage: Symptoms may include fractures, limited movement, and noticeable masses.

Diagnosis 진단

English:

  1. Physical examination: Initial examination to detect signs of bone abnormalities.
  2. Imaging: X-rays, MRI, CT scan, and bone scans to visualize the tumor and assess its extent.
  3. Biopsy: Core needle biopsy or surgical biopsy to obtain tissue samples for histological examination.
  4. Blood tests: Including alkaline phosphatase (ALP) and lactate dehydrogenase (LDH) levels.

TNM Stage 병기

Staging Groupings

Combining the T, N, and M classifications, osteosarcomas can be staged as follows:

Stage I

  • T1, N0, M0

Stage II

  • T2, N0, M0
  • T1, N1, M0

Stage III

  • T3, Any N, M0
  • T1-T2, N1, M0

Stage IV

  • T4, Any N, Any M
  • Any T, Any N, M1

Treatment 치료

English:

  1. Surgery: Primary treatment to remove the tumor, often followed by limb-sparing surgery or amputation.
  2. Chemotherapy: Used preoperatively (neoadjuvant) to shrink the tumor and postoperatively (adjuvant) to kill remaining cancer cells.
  3. Radiotherapy: Less commonly used but may be considered for certain cases or palliative care.
  4. Targeted therapy: Drugs that target specific molecular pathways in cancer cells.
  5. Rehabilitation: Physical therapy to help restore function and mobility after surgery.

Prognosis Based on TNM Stage

English:

  1. Stage I: Generally, the prognosis is favorable with a high survival rate if the tumor is localized and completely resectable.
  2. Stage II: Prognosis varies; better if localized and resectable.
  3. Stage III: Prognosis is poorer due to local invasion and potential spread to lymph nodes.
  4. Stage IV: Prognosis is generally poor due to distant metastasis and significant spread.

Radiotherapy in Osteosarcoma

English:

  • Dose: Typically ranges from 50 to 70 Gy, administered in fractions of 1.8 to 2 Gy per session.
  • Anatomical Range: The radiotherapy field generally includes the tumor site and may extend to adjacent tissues and bones to ensure comprehensive treatment, particularly if surgical margins are close or if the tumor is inoperable.
